Anxiety – Dr. Jan Philamon

Anxiety:  How often do you find yourself stuck in ‘catastrophising thinking patterns?’ Thinking of the worst possible outcomes, believing something is far worse than it actually is, or anticipating things will go wrong. Yes…It’s nasty old anxiety.

Anxiety is a very common mental health condition in Australia. In a 12-month period, over two million Australians experience anxiety. Anxiety is a common mental health condition, and it can make people feel quite miserable. The good news is that the sooner people with anxiety seek help, the sooner they are likely to recover.

Usually, an anxiety condition isn’t developed or caused by a single factor but a combination of things. Personality factors, difficult life experiences and physical health all play a role. Research suggests that people with certain personality traits are more likely to experience anxiety. For example, both adults and children who are perfectionists, easily flustered, shy, lack confidence in themselves, have low self-esteem or want to control everything, sometimes develop anxiety during childhood, adolescence or adulthood.

Anxiety conditions may develop as a result of stressful life events, such as work place stress or job change, financial concerns, loss of employment, shifting house, family issues and relationship breakdowns. Other more serious events that may lead to an anxiety condition include the death of a loved one, verbal, sexual, physical or emotional abuse and trauma. It is important to remember that everyone is different and it’s often a combination of factors that can contribute to developing an anxiety condition. You can’t always identify the cause of anxiety or change difficult circumstances, the most important thing is to recognise the signs and symptoms and seek advice and support.

Counselling Approach:  There are several types of effective psychological treatments Jan implements for anxiety. One of which is CBT. This approach helps by teaching you to become aware of your thoughts and to consciously think more realistically. If you actively avoid situations or things that cause anxiety, CBT can help you face your fears and approach these situations more rationally and calmly. CBT can help you recognise the difference between productive and unproductive worries and teach you to let go of those negative and destructive thoughts.

Jan can also help you use relaxation, visualisation and breathing techniques, particularly muscle relaxation, to control anxiety and the physical symptoms of tension.

Trauma Counselling – Susanne Gilmour

Trauma Counselling: Trauma is often defined as complex, repeated incidents of abuse and horrific events in childhood, in relationships, or in war zones.  PTSD is the traumatic response, either to a single or several discrete yet shocking events. This response may overwhelm an individual’s ability to cope with fear, horror and pain in an effort to avoid any reminders or memories of the event/s. In doing so, individuals can often suffer from  some or all of the following symptoms: nightmares; flashbacks; hypervigilance; angry outbursts; substance abuse; anxiety; depression; dissociative symptoms.

Counselling Approach: The initial session of trauma counselling involves getting to know a client’s history and establishing a comfortable environment where she/he feels a sense of safety.  For badly traumatized clients this may take several sessions.  Susanne can then move on to some psychoeducation about trauma and simple explanations of the effects on the mind, how the brain can change through therapy to reduce and even eventually eliminate the disturbing symptoms. Susanne takes great care to understand the personality and interests of all her clients, weighing these against the intensity of trauma symptoms, in order to design the therapy plan best suited to the client.  Initially, self-calming strategies based on a neurological understanding of the stress response are taught.

Susanne considers listening to the client to be of paramount importance, as well as the continuous incorporation of physiological techniques to down-regulate the stress response and increase cognitive functioning.  This work may include Trauma Focused CBT, Acceptance and Commitment therapy, Schema Therapy, Emotion Freedom Techniques (incorporating eye movement and acupressure points). Susanne works to adapt the therapeutic techniques to suit each individual, depending on the client’s  values,  lifestyle and preferred mode of learning. Susanne always encourages her clients to deepen their understanding with recommended readings or electronic resources according to their preference.

Grief and Loss: as human beings we all experience grief and loss during our lives. Some people cope with this experience better than others. We all go through our grief process in our own way.

Counselling Approach: I always think it is very important to just be with a grieving client. Therapy really is about assisting the grief process and helping the client to not be stuck in their grief. I always try to empathise with what the client is experiencing.

Relationship Counselling: when we are in a stressful relationship situation it can have a detrimental effect on our overall well-being. Such conflicts, role disputes, work place situations and generally any relationship dealings that are not going well cause this stress.

Counselling Approach: the basic approach to relationship work in a therapy session is Interpersonal Therapy. But other methods of solving relationship issues can and will also be used: solution focussed, brief focussed, role-playing. Cognitive Behavioural Therapy – Dr David Wells

Cognitive Behavioural Therapy: Perhaps the best known of the therapy approaches used by psychologist. Basically it is the identification of types of thinking and ways of behaving that are not doing as much good to the client as they could do and then learning ways of changing those thoughts and behaviours.

Play Therapy – Joey Tai

Joey Tai     Quals: BA Psych (hons), Master of Clinical Psychology.

Play Therapy: Children who experience emotional or psychological distress are often hesitant, unwilling or simply unable to communicate their innermost fears, insecurities or emotions. In my experience, I find that children who attend an appointment with a psychologist seem to assume that they are either meeting with an authority figure (such as a principal or police officer) or a medical physician (such as a dentist or GP). As such, they are nervous, fearful and reserved. Play therapy is an excellent form of therapy as it helps to recreate a safe environment for the child, in which he/she can comfortably “be themselves”.

Counselling Approach: Counselling Strategy – Once in this safe and secure environment, I am then able to observe and understand the underlying dynamics that the child experiences in his relationships with his family and friends. The child is open to a more ‘natural’ inquisition to his/her inner world and true shifts in perception can occur. I am then also able to give feedback to the parent on what intervention or changes are necessary to help the child.

Cognitive Assessment – Susanne Gilmour

Cognitive Assessment: A  cognitive assessment assists in explaining why a child may be struggling or underachieving at school.  It involves the administration  of an IQ test measuring potential academic ability . If  any strengths and weaknesses present they may indicate that one or more specific learning disabilities rather than an intellectual disability  may explain  the student’s difficulties.  Then  an assessment of achievement is required ,and possibly assessments of memory, executive functioning and/ or social communication skills.

Counselling Approach: Ms. Gilmour has been a teacher and school psychologist for 20 years, counselling, consulting on special needs  and  carrying out educational assessments in primary and secondary schools for 5-17 year olds.  She also provides cognitive assessments for 18 years and up. Meeting the child’s caregivers first is important to gather a comprehensive history of the child’s functioning in different settings and over time and ruling out trauma as a possible explanation of some of his/her difficulties.

Relaxing the person  at the start of assessment  is essential in getting best performance as is the requirement that assessment be carried out on a day when the child is kept home from school as the student is likely to be experiencing  ‘cognitive fatigue’ if she/he has attended school on the day testing occurs.  Initial assessments may indicate further testing of memory will be required.   Take home parent and teacher standardized surveys of executive functioning and social communication skills may also be necessary to arrive at an accurate diagnosis.

Ms. Gilmour writes reports that help parents to assist their child and learn which specialists (paediatrician, speech pathologist and or audiologist to assess auditory processing) will further advise and provide reports that may allow the school to access funding to support their child.  Teachers are given recommendations on educational strategies that help to maximize the student’s learning potential.
